Disease and treatment history
- Current EDSS score: 3.5
- Currently on third therapy for 5 years. She’s been stable, but is manifesting signs of increased vulnerability to infection
- In the past year, she is reporting more frequent UTIs and a skin infection.
Disease treatment & history
- Prior to her current anti-CD20 therapy, she was on an injectable DMT and then an oral DMT. Each therapy was discontinued due to disease progression
- Diagnosed 15 years ago after experiencing weakness in her right leg
- MRI scans revealed multiple brain lesions
Recent disease activity
- Relapses: No discernable relapse
- MRI: Stable the past 2 years with areas of T2 hyperintensity in the brain and spinal cord
- Disability progression: Can no longer walk as far as she used to and bladder symptoms getting worse
Clinical considerations
- Concerned about immunosuppression and frequent infections with current medication, but worried about decreased efficacy if she switches treatment
- Struggling with brain fog (e.g., finding right words in conversations)
Non-clinical considerations
- Looking forward to traveling with her husband and does not want to be hindered by frequent UTIs or respiratory infections
- Concerned about insurance coverage of her current DMT when she retires
- Getting tired of frequent doctor appointments for infections
